This page covers the regulatory authority, device classification, registration process, local representative requirements and FAQs for medical device registration in Russia. Medical devices in Russia are regulated by the Federal Service for Surveillance in Healthcare (Roszdravnadzor), and EVERBIZ can support you through the entire process from classification to registration.

1. Regulatory Framework
Medical devices in Russia are regulated by the Federal Service for Surveillance in Healthcare (Roszdravnadzor / RZN). Russia is a member of the Eurasian Economic Union (EAEU) (together with Belarus, Kazakhstan, Armenia and Kyrgyzstan), resulting in two parallel registration systems: national and EAEU.
Core regulations:
-
Government Decree No. 1684/2024 – in force since 1 March 2025, replacing Decree No. 1416/2012 which had applied for over a decade; the new main framework for national registration in Russia
-
Ministry of Health Order No. 181n (11 April 2025) – in force since 1 September 2025, significantly revising requirements for technical and operational documentation
-
EAEU Agreement on Common Principles and Rules for the Circulation of Medical Devices (in force since 2016; transition period extended several times)
2. Risk Classification
Russia (and the EAEU) uses a four-tier risk classification consistent with the EU:
| Class | Risk | EU MDR equivalent | Indicative registration time |
|---|---|---|---|
| Class 1 | Low | Class I | About 50 calendar days |
| Class 2a | Low–moderate | Class IIa | About 90 calendar days |
| Class 2b | Moderate–high | Class IIb | About 150 calendar days |
| Class 3 | High | Class III | Up to 2 years |
In practice, the overall timeline for importers is usually 10–16 months (including deficiency responses, testing, clinical work, etc.).
3. Two Registration Routes
Route 1: National registration (Roszdravnadzor)
-
Valid only in Russia
-
Based on Decree No. 1684/2024
-
Registration certificates are valid indefinitely, unless significant changes are made to the device’s design, intended use or manufacturing
-
Deadline for new applications: 31 December 2026
Route 2: EAEU registration
-
One certificate for 5 national markets
-
The manufacturer selects a Reference State for a single submission
-
Testing and clinical studies can be performed by accredited bodies in any EAEU member state
-
Manufacturing site inspections are organized by the Reference State
-
From 1 January 2027, new applications can only use the EAEU route
4. Registration Process
Step 1 — Appoint an authorized representative
-
National registration – must be a legal entity or sole proprietor in Russia
-
EAEU registration – can be an entity in any member state
-
The POA must be apostilled (for documents from the EU) or consularly legalized
Step 2 — Prepare documents
-
Technical file (can be adapted from EU MDR Annex II to Russian/EAEU requirements)
-
GOST ISO 13485:2016 certificate – mandatory for sterile Class 2a, Class 2b and Class 3
-
Risk management report (ISO 14971)
-
Russian versions of the IFU and labeling – EAEU registration also requires the official languages of all target countries
-
Color mock-ups of packaging and labels (new requirement under Order No. 181n)
-
Clinical evaluation report
Step 3 — Local testing: Technical testing, toxicological/biological evaluation, electrical safety, etc. are carried out in accredited Russian laboratories.
Step 4 — Clinical evaluation / clinical trials
-
Literature review route (for products with sufficient foreign data)
-
Local clinical trials (high-risk or novel devices)
Step 5 — Manufacturing site inspection (QMS inspection): Since January 2024, manufacturing site inspections are mandatory for sterile Class 2a, Class 2b and Class 3 devices.
Step 6 — Roszdravnadzor registration review: Once approved, a Registration Certificate is issued.
5. New Requirements under Order No. 181n (Effective 1 September 2025)
Order No. 181n comprehensively upgrades the requirements for technical and operational documentation:
-
Segmented by device type: different requirements for conventional devices, software and AI software
-
Mandatory color packaging/label mock-ups
-
Simplified user manuals – Class 1 and 2a may use a simplified version (intended use, basic specifications, contraindications, operating rules and manufacturer information only)
-
Strengthened V&V documentation – laboratory tests must simulate real conditions of use, including animal studies where necessary
-
Life-cycle documentation – maintenance plans, spare parts catalogues, repair documentation, disposal and environmental safety
-
Specific requirements for IVDs
Specific requirements for software medical devices
-
Mandatory version control and update procedures
-
A mandatory cybersecurity section: authentication, encrypted storage and transmission, access control, security auditing, incident response
-
AI devices: must automatically transmit algorithm outputs and performance data to Roszdravnadzor’s automated information system (AIS)
6. Post-Market Surveillance
Mandatory obligations
-
Batch-by-batch import notification – Roszdravnadzor must be notified within 15 days of each imported batch
-
Class III and implantable Class IIb devices – under Regulation #1113, devices registered since 2021 must submit an annual post-market clinical follow-up report
-
Adverse event reporting and field safety corrective actions
-
Changes of registration certificate holder must be updated promptly, otherwise the certificate becomes invalid
Risk-based supervision (new rules of 28 November 2025)
Roszdravnadzor applies five-level risk-based supervision to market participants (manufacturers, importers, distributors):
| Risk level | Applies to | Inspection frequency |
|---|---|---|
| High | IVD manufacturers, Class 3 implant manufacturers, clinical research institutions | One inspection every 2 years or annual preventive visits |
| Significant | Large manufacturers/importers with a record of violations | Relatively frequent |
| Medium | Moderate-risk devices, no violations | Moderate |
| Moderate | Low-risk devices | Lower |
| Low | Small operators, simple products | No mandatory preventive visits |
Supervision tools have been formally digitized: video-conference inspections and remote supervision via the Inspector mobile app. “Mystery shopping” checks require prosecutor approval.
7. Recent Key Changes
| Date | Change |
|---|---|
| January 2024 | Manufacturing site inspections mandatory for sterile Class 2a, Class 2b and Class 3 |
| November 2024 | Decree No. 1684 issued, replacing the 2012 decree |
| 1 March 2025 | Decree No. 1684 takes effect; new national registration rules apply |
| 1 September 2025 | Order No. 181n takes effect, significantly upgrading technical documentation/software/AI requirements |
| 28 November 2025 | Five-level risk-based supervision introduced; remote supervision formalized |
| 31 December 2026 | Deadline for new national registration applications |
| 1 January 2027 | New registrations only via the EAEU route |
8. Customs and the EAC Mark
Medical devices circulating within the EAEU must comply with the relevant EAEU technical regulations and bear the EAC conformity mark (for certain categories). Under the EAEU system, EAC compliance and device registration are managed in parallel.
Strategic Value and Challenges
Opportunities:
-
One certificate, five markets – EAEU registration covers Russia, Belarus, Kazakhstan, Armenia and Kyrgyzstan
-
Registration certificates are valid indefinitely, keeping long-term compliance costs under control
-
Classification aligned with the EU MDR, so CE technical files can be largely reused
-
Fast track for products affected by sanctions – Decree 1684 introduces an expedited registration mechanism
Challenges:
-
Mandatory local testing, Russian-language documentation and cumbersome apostille procedures
-
Order No. 181n’s cybersecurity and data reporting requirements for software/AI devices are significantly stricter than in most markets
-
The EAEU route becomes mandatory from 2027, leaving a limited transition window
-
Complex logistics, payments and partnerships amid international sanctions
-
High-risk devices may face annual preventive inspections
